What Causes Lambert–Eaton?

Lambert–Eaton myasthenic syndrome involves the production of IgG antibodies against the presynaptic P/Q voltage-gated calcium channels (Cav2.1), leading to a decreased release of calcium-dependent Ach from the presynaptic membrane. This decrement in the amount of Ach release will result in a reduced end-plate potential (EPP) below threshold on the postsynaptic membrane, thereby blocking the transmission at the NMJ [34].

The antigenic sites on the voltage-gated calcium channels bear a resemblance to the cell lines from small-cell lung carcinoma, and other neuroendocrine tumors. Glial nuclear antigen, a transcription factor spotted in patients with both SCLC and LEMS, but was absent in LEMS alone [8].
